Management.
Key Responsibilities
Assess beneficiaries to identify medical, physical and psychosocial
needs
Collaborate with the clinical team to provide whole-person care and
support chronic condition management
Facilitate access to medical and behavioral resources while
ensuring adherence to standards of care
Required Qualifications
Current, unrestricted LCSW license (BSW acceptable)
U.S. citizenship
Minimum of two years of experience in case management or related
fields
Availability to work in Central and Mountain Standard Time
zones
